EXPENDITURE ESTIMATES FOR 2003 AIDS PROJECTS IN HARARE AND KINSHASA (April 2003)

The following figures represent a forward estimate calculated in early April 2003 for the 2003 AIDS projects in Harare and Kinshasa. They provide the basis for the sum of US$90,438 that Compassion Response Network is seeking to raise through public donations.

Actual expenditure will naturally vary from these forward estimates. Some items will be cheaper and others more expensive than the estimates provided. All money donated must be officially recorded by either an official receipt (sent to donor if over AUD$5 and received by post) or through the bank statement (if donated through direct deposit to the CRN account). These moneys received will be strictly balanced against all actual expenditures and the accounts will be subject to annual audit control under strict Australian accounting standards.

The AIDS projects are estimated to require about 8 months to conclude, and by the time of conclusion, the amount needed may be different from our initial estimates.

Purchases will be made in US dollars (in Kinshasa), in Zimbabwe dollars (in Harare), in South African Rand (for purchases in South Africa) and Australian dollars (for purchases in Australia). The US dollar is the currency to naturally work from in our overall estimates. In these estimates we are working off official exchange rates as of 19/March/2003;

AUD$1.00 (Australian) = US$0.5917.

The value of the Zimbabwe dollar is fluctuating wildly against the US dollar, the Zimbabwe dollar having devalued about three-fold in the past 8 months. We are working off an exchange rate for August/02 of US$1 - Zim$400 and March/03 of US$1 = Zim$800. No clear estimate can be predicted for the actual exchange rate for the Zimbabwe dollar on any particular day, as from day to day the extremes of fluctuation might almost double each other.

Some in Zimbabwe (those with jobs fortunate to be so provided by their employer) are covered by medical aid, which covers some medical expenses. In our Harare project we have therefore selected 14 (half) of the 27 AIDS patients receiving full blood tests from those who are on medical aid; this reduces our medical expenses. The remainder of our AIDS patients in Harare are selected from the majority without medical aid. In Kinshasa there is no provision for medical aid or health insurance.

The estimates for the Harare project and the Kinshasa project will be seen to vary in many ways, not just in accordance with the number of patients treated in the project at each location. The Congo for example has seen desperate poverty and negligent government for many generations, and the Congo is now a highly indebted Third World Nation.

In contrast, the Zimbabwe economy is rapidly collapsing because of the political/economic troubles in that nation. But it is coming from a semi affluent British ruled history, in which businesses and technology have developed somewhat, and a commercial infrastructure has been somewhat established. You will find therefore within Zimbabwe a situation of acute food and petrol shortages, and yet affluent commodities like cars and medical laboratory tests are cheaper and more available than in the Congo.

In both Zimbabwe and the Congo, the poor are extremely poor, and unemployment is extreme in both countries. The overlay of about a third of the adult population infected with HIV creates a horrendous scenario that it remains difficult to imagine for anyone living in an affluent country.

You may wonder why we have not focussed our trials in South Africa where costs for such trials would be cheaper, though the need with respect to AIDS equally as great. The answer is that despite extensive outreach throughout goodwill, compassion and NGO groups in South Africa, we could find no group interested in collaboration with us.

PUBLIC APPEAL BUDGET FOR KINSHASA AND HARARE PROJECTS --- FINAL ESTIMATES

In Kinshasa 7 advanced AIDS patients will be on our treatment projects, all receiving full blood tests, the results of which will be published on the CRN website;

In Harare 27 advanced AIDS patients will receive full blood tests and these results will also be openly published. Nine alternative treatments will be provided to three patients for each treatment;

As an expression of outreach to the many in Africa whom we plan to reach out to in future years, we are providing treatment, a daily meal and iron/folic acid supplementation for 9 more AIDS patients in Harare, though these 9 will not be sponsored for blood tests or be included in published results.

1. MEDICAL TESTING

2-monthly tests

7 AIDS patients in Kinshasa and 27 AIDS patients in Harare will all receive bDNA viral load, CD4 immunity test and full blood count;

The bDNA test counts the number of active HIV viruses in a blood sample,
The CD4 test provides an indicator of the health of the body's immune system,
Full Blood Count counts white blood cells + red blood cells + platelets in a blood sample.

Harare

For Harare the 27 selected patients we calculate for 14 on medical aid and 13 are not. bDNA tests are not covered by medical aid. One CD4 test every 6 months (2 tests in all) are covered by medical aid. Full blood count tests are covered by medical aid.

Kinshasa

Viral Load or CD4 counts are not available in Kinshasa. We will be arranging private courier deliveries of blood samples from Kinshasa to Johannesburg every two months.

Toga Laboratories in Johannesburg offers a concession rate for Viral Count (bDNA) + immunity count (CD4) + Full Blood Count at Rand 815.00 for one sample of a patient.

For one patient concession profile = Rand 815.00 = US$ 100.50
For one delivery of 7 samples = Rand 5,705 = US$ 703.50
Cost for a courier delivery to Johannesburg of 7 samples will be US$ 350.00.

So total cost for courier and laboratory testing
= US$703.50 + US$350.00 = US$1,053.50 every two months.

If we are to consider the overall cost for all 4 deliveries from Kinshasa (courier + blood tests) the overall figure is
US$1,053.50 x 4 = US$4214.00

Bone Marrow Biopsy Tests: Harare

Many treatments, whether herbal or electrical seem to have a primary effect of being an anti-viral agent for the blood stream. That is, if taken regularly over a long time they gradually reduce and in time eliminate the HIV in the blood stream. But when the treatment is stopped, the HIV re-infects because the treatment did not heal HIV infections deeply lodged in bone marrow and brain cell tissues. So when these cells erupt the blood stream is re-infected.

The important test to discern whether a patient is really cured long term will be to do a bone marrow cell biopsy on our selected patients before treatment and after the 6 months of treatment. And then look for HIV viral load levels in the biopsy tissue. That will indicate if the treatment is really enduring.

Because of the high cost involved, we are providing bone marrow biopsy with bDNA assessment of HIV viral load, before treatment and after the six months of treatment, to just one patient on each of the nine treatments in Harare. No bone marrow biopsies will be conducted in Kinshasa.

The operation cost plus bDNA assessment comes to about US$160 per biopsy.

Of these 27 patients, the 14 patients on medical aid receive these tests free. We will be treating 13 selected patients not on medical aid, each having this set of tests done twice, once pre-treatment and once after 6 months of treatment.

There is no medical aid coverage in Kinshasa. These tests are available in Kinshasa, though because of the poor development of medical services in that city they are more expensive than in Harare. Each patient will require the tests done twice.

Oxygen Testing: Harare

Because of the potency of pure colloidal silver for the three patients receiving pure colloidal silver + Oxyrich, we will be checking the oxygen level in the blood three times, before treatment, and at the two weeks and two months stages, to check whether oxygen level in the blood is remaining normal.

Total cost for Harare project = US$6.75 x 3 patients x 3 times =US$60

2. MEDICAL ITEMS

Costs are very much reduced if Compassion Response Network sponsors costs for various medical items used by doctors in taking blood samples and otherwise

EDTA tubes for transporting blood samples internationally and sample kits have already been purchased and do not need to be included in future estimates. The remaining items are being bought in Australia and delivered by private courier.

Ferrous Sulphate

Iron supplementation is important because many AIDS patients suffer from anemia. We provide 325mg ferrous sulphate tablets to each of the AIDS patients (36 in Harare + 7 in Kinshasa) three times a week for twenty weeks (60 tablets each patient). Iron also needs to be provided in a nutritious diet.

Harare

36 bottles of 30 x 325mg ferrous sulphate tablets have already been purchased for Kinshasa. 48 more bottles ferrous sulphate for Harare need to be purchased in Australia for AUD $256.80 = US$154.00

Kinshasa

14 bottles of 30 x 325mg ferrous sulphate tablets have already been purchased for Kinshasa. No more ferrous sulphate purchases are required for Kinshasa.

Folic Acid

If a patient receives ferrous sulphate, he also needs folic acid which is important for iron absorption. Each patient will receive a 2 1/2 mg Folic Acid tablet three times a week for the first 20 weeks of the treatment. Folic Acid will be purchased in Australia where it is cheap.

For Harare: 13 bottles x $5 = AUD$65 = about US$39

For Kinshasa 3 bottles x $5 = AUD$15 = about US$9

Total = AUD$80 = about US$48

3. TREATMENT HOUSES

Harare

Two treatment houses will be used in Harare, one in the high density area so to be close to the homes of the AIDS patients and not require enormous costs for transport, and one at PNI Headquarters, in a medium density area.

4. OFFICE

Harare computer and office

PNI Empowerment Trust during its early years of establishment in 1998 and 1999 was self-sufficient, while having an immense outreach and service to AIDS patients and popularity throughout Zimbabwe. Those who could afford donated or paid for courses and books, and it supported a small crafts industry. In the past two years however, with the collapse in the Zimbabwe economy, PNI Empowerment Trust found it more and more difficult to attract donations, while the costs of living in Harare skyrocketed. As a result towards the end of 2002, PNI Empowerment Trust was on the point of financial collapse. This would have been a tragedy because it has such a positive outreach to thousands of AIDS sufferers within Zimbabwe. Consequently, apart from sponsoring the costs of the CRN AIDS projects in Harare, CRN decided to sponsor the continuing work of PNI Empowerment Trust at least throughout the trials period of 2003.

This sponsorship would require, apart from the costs of the various trials, hiring a secretary and office assistant for the PNI office, providing US$2,500 worth of office equipment for PNI office for computers and other equipment, together with PNI monthly office expenses throughout the trials period.

PNI Empowerment Trust has listed a host of hardware items it requires for its office; a surge protector, a medium-range computer, a second hand 486 computer, desks, scanner, guillotine, digital camera, a DSTV decoder and many other items.

CRN decided to sponsor US$2,500 for purchase of the needed office equipment. This will only meet half the need, though PNI can choose which items are most important to purchase.

As some will receive meals for 8 months of the project, the food estimate is based upon an average of 7 months food for all.

CRN is sponsoring the return air fare from Harare to Johannesburg for Geraldene the co-ordinator of the Harare project to go to Johannesburg. This trip will have a number of purposes related to the overall CRN sponsored AIDS work in Harare;

      To pick up various medical items, computer and office hardware and other items essential for the Harare project,

      To make vital networking contacts to expand the AIDS work

9. Contingency Reserve

US$5,000 has been agreed to be set aside as a contingency reserve for unplanned expenses. Many items will be overlooked in the initial planning process, and this may well be an underestimate of the eventual need. African service groups will need to have funds available for emergency use, and so this reserve will initially be distributed in this way;

US$1,000 be available immediately for use by the Kinshasa service group,
US$2,500 be available immediately for use by Harare PNI,
US$1,500 be held in reserve subject to management group resolution.
